     You are about to initiate a Prequalification Risk Assessment Survey as a potential research partner with NJDOT's Bureau of Research.  Please enter your university/organization's name and a valid email address to which you have access.  Clicking the SUBMIT button will then generate a University/Organization Identification Number (UOID), sent to you via that email address. 

     Within that email will be a link to the survey itself.  Please highlight and cut your UOID number from the email, click the survey link, and paste it in the indicated field on the presented form.  Thank you!
